Choosing Among Types Of Fehb Plans

You can choose from among managed fee for service plans, regardless of where you live, or plans offering a point of service product and health maintenance organizations if you live within the area serviced by the plan. You will find managed care features in all the plans. Common features of managed care are pre-approval of hospital stays, the use of primary care providers as gatekeepers to coordinate your medical care, and networks of physicians and other providers.

In prepaid plans, your covered health services are pre-funded by your premium and the governments contribution toward the cost of your health insurance. Generally you must use specified plan physicians, hospitals and other providers at designated locations, although care elsewhere may be available after a referral.

FFS plans reimburse you or your physician or hospital for covered services rather than provide or arrange for services as prepaid plans do. FFS plans allow you to choose your own physicians, hospitals and other health care providers without a referral. Some are open to all enrollees, but others require that you join the organization that sponsors the plan. Some plans limit enrollment to certain employee groups.

Public Service Loan Forgiveness Program

Through the Public Service Loan Forgiveness Program, the government forgives the remaining balance on eligible student loans for people who have worked in a public service job for at least 10 years.

To qualify, program applicants must have already made 120 monthly payments and be employed full time in AmeriCorps, the Peace Corps or another public service organization such as:

  • The federal government or a state or local government.
  • A public child or family service agency.
  • A 501 nonprofit organization.
  • A private organization that provides public safety, public interest law services, public health, law enforcement or another a public service.

How Does Medicare Work

With Medicare, you have options in how you get your coverage. Once you enroll, youll need to decide how youll get your Medicare coverage. There are 2 main ways:

Original Medicare

Original Medicare includes Medicare Part A and Medicare Part B . You pay for services as you get them. When you get services, youll pay a

deductible

at the start of each year, and you usually pay 20% of the cost of the Medicare-approved service, called coinsurance. If you want drug coverage, you can add a separate drug plan .

Original Medicare pays for much, but not all, of the cost for covered health care services and supplies. A Medicare Supplement Insurance policy can help pay some of the remaining health care costs, like copayments, coinsurance, and deductibles. Some Medigap policies also cover services that Original Medicare doesn’t cover, like emergency medical care when you travel outside the U.S.

Data And Evaluation Systems

When evaluating health services, the United States is both data rich and poor. Compared with health systems where there is a single payer, U.S. data are divided among many insurers, making it virtually impossible to produce comprehensive provider or beneficiary profiles. However, the United States also requires detailed diagnostic and procedural information on each bill paid in an FFS system. Moreover, hospital admissions and major surgery often require preadmission review. Consequently, a great deal of information that is unavailable in systems which do not require detailed bills is produced at the patient level.

Various systems to improve data and to coordinate data systems are under development. The Institute of Medicine of the National Academy of Sciences has issued several reports calling for more health outcomes research and improved data systems, including computerized patient records . The Federal Government publishes uniform mortality statistics for hospitals based on Medicare billing records and quality information on nursing homes based on periodic inspections. The Medicare program is also developing a uniform clinical data set to evaluate the quality of care and outcomes of Medicare patients. The Federal research effort on medical outcomes, including the development of medical practice guidelines, is coordinated by the Agency for Health Care Policy and Research.

Types Of Health Plans

Because there are many different types of health plans, you should be sure to look for the one that fits your needs. Comprehensive health insurance provides benefits for a broad range of health care services. These health plans offer a detailed list of health benefits, may limit your costs if you get services from one of the providers in the plans network, and typically require co-payments and deductibles.

Here are some of the types of plans offered in Massachusetts

Health Maintenance Organization

HMO plans cover hospital, medical and preventive care. You are only covered if you get your care from HMO’s network of providers . With most HMO plans you pay a copayment for each covered service. For example, you pay $30 for an office visit and the HMO pays the rest of the cost.

Preferred Provider Plans

Preferred provider plans usually cover hospital, medical and preventive care. These plans have a network of preferred providers that you can use, but they also cover services for out-of-network providers. PPP’s will pay more of the cost if you use a provider that is in the network. Example: After copays and deductibles, the plan pays 100% of a service for a network provider but 80% for an out-of-network provider. Note that if you choose to go OON when you are in a PPP, your provider may balance bill you directly for the entire cost of the procedure.

There are about 6,700 hospitals in the United States, including 5,480 community, acute care hospitals, 880 specialty hospitals , and 340 Federal hospitals open only to military personnel, veterans, or native Americans. Of the 5,480 community hospitals, non-profit hospitals represent 59 percent, local government hospitals 27 percent, and for-profit hospitals 14 percent. There are 3.9 community hospital beds for each 1,000 residents, although this varies around the country. There were 33 million hospital admissions in 1990 with an average length of stay per admission of 9.2 days. Hospital stays are shorter and admission rates are lower in the United States compared with other OECD countries. The average hospital occupancy rate, 66 percent, is lower in the United States than in other OECD countries, however this rate varies and may be 40 percent or lower in rural areas .

Hospitals finance capital purchases through a variety of means including savings, tax-exempt bond issues, and philanthropy. Although Federal and State mortgage loan guarantee programs assist some hospitals to secure financing for construction and renovation projects, it is more common for hospitals to secure private mortgage insurance when floating a construction bond. Some States require prior approval before certain capital projects can be undertaken, while other States have no prior approval procedures.
