If your baby was born in an Ontario hospital with birthing facilities or with a registered midwife, follow these steps to apply:
Your babys new health card will be mailed to you within 8 weeks of the date the Ontario Health Coverage Infant Registration form was submitted.
If hospital staff or the registered midwife gave you the Ontario Health Coverage Infant Registration form and you have questions or concerns about your babys health card, you must contact ServiceOntario.
If hospital staff or the registered midwife do not give you the Ontario Health Coverage Infant Registration form, you may apply for health coverage for your child at either:

If your baby was born in an Ontario hospital without birthing facilities or without a registered midwife, you must apply in person at the nearest ServiceOntario centre:

- if you go more than 90 days after the birthdate, bring proof of your childs Canadian citizenship
If your baby was born at home without a registered midwife, you will require the babys Ontario birth certificate or an Ontario Statement of Live Birth for OHIP registration. You must also bring all of the following documents with you to the ServiceOntario centre:

Prepare for Annual Eligibility Recertification
Each year you must certify that you are still eligible to receive the Lifeline Program benefit and that no one else in your household is receiving a Lifeline Program benefit.
Typically by the anniversary date of your enrollment in the Lifeline Program with your provider, you will be contacted by USAC with instructions on how to complete the recertification process. You must respond to this request within 60 days.
If you do not complete this recertification process, you will lose your Lifeline benefit. This means your monthly bill may increase, or the services you normally receive for free each month will not be provided.
If USAC determines that you are no longer eligible for the Lifeline benefit, it will notify you of such and your Lifeline service will be terminated in 30 days, unless you provide proof of eligibility.
If you become ineligible for the Lifeline benefit at any time, you must contact your Lifeline provider within 30 days to de-enroll from the program or you may be subject to penalties.

This restricts consumers ability to submit enrollment requests for California LifeLine wireless telephone services. The 30-day clock for the enrollment request freeze duration starts when the California LifeLine Administrator generates an application packet for a consumer or confirms that the request is an inter-carrier transfer request. The CPUC adopted three types of activities that would stop the enrollment request freeze duration, which are as follows:
